Yes. CMS first allowed virtual contrast supervision in April 2020, during the COVID-19 Public Health Emergency Policy and Regulatory Revisions. To reduce the risks of exposure for patients and healthcare professionals, CMS revised the definition of Direct Supervision to include “virtual presence through audio/video real-time communications technology” when appropriate.
Since then, CMS has extended this definition each year through the Medicare Physician Fee Schedule (MPFS) Final Rules - 2021, 2024, and 2025. Most recently, in the 2026 MPFS Proposed Rule, CMS has, for the first time, proposed making virtual supervision via real-time audio/video communication a permanent and compliant method of meeting Direct Supervision requirements.

Yes. After a reaction, patients are provided with access to a 24/7 physician hotline. If they have questions or concerns after being discharged, they can speak directly with the on-call physician for immediate answers, clinical guidance, and peace of mind.
Immediate - measured in seconds. As soon as the technologist alerts the on-call physician of a reaction, the physician will connect live via audio and video to evaluate the patient’s symptoms and vitals, while guiding the technologist on next steps. Our supervising physicians are highly experienced in managing contrast reactions of all types. On average, our team treats 5 to 10 contrast reactions each day, ranging from mild extravasations to severe anaphylactic reactions that require EpiPen administration and EMS support. We supervise more than 55,000 contrast exams, and manage more than 130 reactions each month. Our team brings unmatched experience and expertise in contrast reaction management.
Because availability is critical. During a time-sensitive contrast reaction, you need confidence that the supervising physician is fully alert, focused, and immediately ready to respond. Radiologists are uniquely suited for this role, their workflow allows them to pause dictation instantly without the risk of interrupting a patient exam, consultation, or procedure. No other specialty offers this same level of flexibility and availability.
By contrast, physicians from other specialties can only provide virtual supervision when they are off clinical duty, which typically happens when the clinician is post-call and fatigued, or on PTO and mentally disengaged. Neither situation is ideal when seconds count and patients need a physician who is mentally present and ready to act.

Yes. Before going live, technologists complete a two-part contrast reaction training program. The first component is an online course featuring video modules, interactive simulations, and exams that cover safe medication administration and management of adverse reactions. The second component is onsite, physician-led training that includes scenario-based exercises, mock codes, workflow reviews, hands-on practice with medication administration, and a Q&A session. The contrast reaction training program is comprehensive and ensures technologists have the knowledge and confidence to safely administer medications under physician supervision. Training is renewed annually at no additional cost.
Yes. We provide technologists simplified workflows, quicksheets for reference, and real-time support whenever needed. They also have access to a regional nursing team for educational reviews, onsite assistance, and onboarding of new technologists.

No. While CMS guidelines support virtual supervision, not all states permit it. Each state sets its own regulations, and CMS rules do not override state-specific requirements. In states where virtual supervision is not yet permitted, it may be possible to request an exemption from the relevant state agency. We have successfully helped partners navigate this process to obtain special permission to proceed with virtual supervision.
